PLAYGROUND WAIVER

(Hereinafter referred to as "Playground Facility")

1. Acknowledgement of Risk
The Parent/Guardian acknowledges that participation in playground activities inherently involves risks and dangers that may result in injury, including but not limited to physical injury, emotional harm, or death. These risks may include, but are not limited to:

Falling from playground equipment
Colliding with other children or objects
Improper use of playground equipment
Slips, trips, and falls
2. Assumption of Risk
By signing this Agreement, the Parent/Guardian agrees to voluntarily assume all risks and hazards associated with participation in activities at the Playground Facility. The Parent/Guardian understands that injuries or accidents may occur despite safety measures, and agrees to take personal responsibility for any injuries or damages that may occur.

3. Release of Liability
The Parent/Guardian, on behalf of themselves and their minor child(ren) listed below, hereby releases, discharges, and holds harmless the Playground Facility, its owners, employees, agents, and volunteers from any and all claims, demands, causes of action, or liabilities of any nature arising out of or in connection with the use of the Playground Facility, including but not limited to injuries or accidents that may occur during the use of the playground or as a result of equipment failure or negligence.

4. Indemnification
The Parent/Guardian agrees to indemnify and hold harmless the Playground Facility from any and all claims, actions, damages, and liabilities, including reasonable attorney's fees, arising from or related to the use of the playground by the Parent/Guardian or their minor child(ren).

5. Medical Treatment Authorization
In the event of an injury or medical emergency involving the minor child(ren), the Parent/Guardian authorizes the Playground Facility and its employees to provide or arrange for emergency medical treatment as deemed necessary.

6. Parent/Guardian Responsibilities
The Parent/Guardian agrees to supervise their child(ren) at all times while on the Playground Facility premises. The Parent/Guardian acknowledges that the safety of their child(ren) is primarily their responsibility.

7. Severability
If any provision of this Agreement is found to be invalid or unenforceable, the remainder of this Agreement shall remain in full force and effect.

8. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the state in which the Playground Facility is located.
